Medical Requirements
Illinois State Law requires physical examinations, up-to-date immunizations,
and lead screening for all students new to the school and for students entering
Kindergarten and Fifth Grade. Fox River Country Day School must have either
a current Certificate of Child Health Examination or a legal exemption form
on file for each student. Students will be excluded from school if these requirements
are not met by October 1. Students are required by the State of Illinois to
have two measles shots. Children entering school for the first time at the Kindergarten
level and below will be required to show proof of having received one dose of
chickenpox vaccine on or after their first birthday. A physician can confirm
past disease history in lieu of vaccination by having examined the child, documented
the parent’s description of the child’s history, or reviewed laboratory
evidence. A legal exemption form will be accepted in place of the above requirements.
We ask that parents keep children home when they are not feeling well. Please
phone the Main Building Office or Hoxie Office to report absences.
Medication at School
Fox River Country Day has adopted the State of Illinois “Recommended Guidelines
for Medication Administration in the School.” As a regular practice, the
dispensing of student’s medication by school personnel should not occur.
Medication should be given to the child by the parent either before or after
school hours. In exceptional cases, medication prescribed by a physician may
be administered at school on a short-term basis. It must be brought to the Director
of the respective school (Pre, Lower, or Middle) by the parent, accompanied
by a signed and dated Medication Instruction/Release form and must be in a pharmacy
container with the child’s name and valid date on the label. No more than
one dosage can be administered during the school day. No over-the counter
drugs will be administered without prescription.
Emergency Care
If a student becomes ill during the day, or in the event of an accident, parents
will be telephoned immediately (or the emergency contacts telephoned, if parents
cannot be reached). For this reason, parents must ensure that Emergency Forms
are kept up-to-date.
Parents will be notified of any illness or injury that may require follow-up
medical care. A “Statement of Permission and Release Consent” form
and “Permission for Emergency Medical and Surgical Treatment” must
be on file for every student, allowing the school to obtain necessary medical
